Revision surgery addresses a problem with previous implants — hardening, malposition, a dropped fold, rippling, or a wish to change size. Dr Colson adapts the operation to the complexity of the specific problem rather than applying one routine.

For an established grade III–IV capsular contracture, he performs an en-bloc capsulectomy — removing implant and capsule together — usually with an implant exchange and regenerative lipofilling to restore soft-tissue cover and calm the pocket.

Malposition & fold correction

Implant malposition is corrected with scars placed according to areolar position. A dropped inframammary fold is rebuilt with capsulorrhaphy or, in selected cases, an abdominal advancement flap to re-establish a stable fold.

How does Dr Colson treat capsular contracture?
For a firm, established contracture (Baker grade III or IV), Dr Colson performs an en-bloc capsulectomy — removing the implant together with its surrounding capsule — usually combined with an implant exchange and regenerative lipofilling, which adds soft-tissue cover and has an anti-inflammatory effect on the pocket.
Do breast implants need to be replaced on a fixed schedule?
Implants do not have a fixed expiry date and do not need routine replacement if there is no problem. Revision is driven by a specific issue — contracture, malposition, rippling, a change of size, or a device concern — rather than by the calendar. Regular follow-up is nonetheless advised.
How are implant malposition and a dropped fold corrected?
Malposition is corrected with scars placed according to the position of the areola. A dropped or poorly defined inframammary fold is repositioned using internal capsulorrhaphy (reinforcing sutures) or, in selected cases, an abdominal advancement flap to rebuild a stable fold.
What role does fat grafting play in revision surgery?
